In accordance with Texas Health and Safety Code Sections 181.103 and 181.105, patients are entitled to receive information regarding how to obtain copies of their medical records, contact the appropriate licensing authority, and file a consumer complaint.
Requesting Medical Records
To request a copy of your medical records, please submit a written request to our office. Requests may be delivered in person, by mail, fax, or email.

Filing a Consumer Complaint
Consumers wishing to file a complaint against a chiropractor may contact the Texas Board of Chiropractic Examiners using the contact information above or submit a complaint online through the Board’s website.
